You also have a right to choose a different provider for any braces or medical devices other than Protect 3D or Stridelink.

3. You will not be treated differently by your physician if you choose to obtain health care services at a facility other than Tattnall Hospital of Reidsville, Georgia – East Georgia Regional Medical Center, Statesboro, Georgia — Optim Surgery Center, Savannah, Georgia – Premier Surgery Center, Brunswick, Georgia – Optim Orthopedics MRI, Statesboro, Brunswick, and Savannah Georgia and Optim Therapy. You also have a right to choose a different provider for any braces or medical devices other than Protect 3D or Stridelink.
